If a user calls in reporting that they are having trouble accessing a Dynamic Form then first test the following troubleshooting steps.


  1. Check if they are trying to use a NWFSC account or a personal account to access the form. If it is a NWFSC account they need to be using SSO to sign in. If it is a personal account they will need to reach out to NextGen at [email protected] for help with their account. (The rest of these instructions assume that they are using an NWFSC account.
  2. Have the user sign into RaiderNet, then have them open another tab and direct them to df.nwfsc.edu. They should reach a page that looks like this:
    This will help ensure that they are capable of signing into their DF account.
  3. Check where the user is getting their link to the form from. There are different links to the same form for SSO users and non-SSO users. If they are getting the link from an email or something of that nature they may have the incorrect link. Direct the user to find the form they are trying to from https://www.nwfsc.edu/raider-central/forms/
  4. If the link from step three did not work, reach out to Temmy Fothergill at [email protected] or on teams and she can generate the needed link using her admin access on Dynamic Forms. 
  5. If all this fails, have the user submit a ticket and assign it to Temmy Fothergill.
